Introduction: Zygomatic complex fractures are some of the prevalent Maxillofacial injuries that are usually caused by Road Traffic Accidents (RTAs), assaults and other violence, falls, and sports.” These fractures may cause some functional and cosmetic issues that require early identification and treatment. Objectives: The objective of this research is to analyze the prevalence and etiology of zygomatic complex fractures in patients who attended DHQ Hospital Karak KPK, Pakistan from January to June 2024. Materials and Methods: A cross sectional study was conducted on 200 patients with zygomatic complex fracture which was confirmed clinically. The collected information includes the demographic data, causes of fractures, the types of fractures, and the methods of handling the case. Results: RTAs were the most common cause (58%), followed by interpersonal violence (22%), falls (15%), and sports injuries (5%). Most cases (64%) involved isolated fractures, and 78% required surgical intervention. Conclusion: Strengthening road safety measures, enforcing helmet use, and implementing violence prevention strategies can significantly reduce the incidence of zygomatic fractures.
